If you have already purchased the test online from our website, you have two options to access your requisition form –
- Email: Search your inbox for a notification from LifeLabs containing your order number, and a link to download the requisition, and follow the next steps for completing your test.
- Order status: Click on order status in the top right corner and sign in by entering your order ID, email address, and one-time passcode that you had set before ordering the test.
If you already have your personalized requisition, please print it, and bring the requisition to your healthcare provider for signature.
